The DHS Forms server contains many DHS forms and publications in Microsoft Word, Excel and Adobe Acrobat Reader® formats.
A number of forms are available in languages other than English, as well as in large print.
Our Forms search page offers many options for finding current and past DHS forms. When using the search page:
Requesting revisions or a new form
DHS employees who would like to have an existing form revised, or a new one created, should submit their request to the DHS Office of Communications Resources, using the DHS 1001 Project Request Form (Word). OCR will evaluate the request and contact the customer to begin production.
DHS staff can order forms and publications in quantity using the Forms Branch Ordering System (FBOS). There should be a designated person in each DHS office who is responsible for ordering forms through this system.
