Filling out the General Request Information Section
Note: All fields labeled in red are required fields and must have at least one entry.
1. Name -- Type your name exactly as it appears in GroupWise (i.e. if your entry includes your middle initial, type it in here; check your InBox or Notify heading if you're unsure). The Help Desk will notify you when your request has been processed.
2. RACF ID -- Type in your DHS mainframe RACF ID. This field helps to further identify you and enables a quicker search in the database if there are questions.
3. Phone Number -- The field is self-formatting so you need only enter the numerals of the phone number without any marks of punctuation or letters (i.e. parentheses, hyphens, and ext (or x, etc.)) Make sure you use standard order -- area code, phone number, extension.
4. Effective Date -- Type in the date you need this to happen (e.g. the date the user starts in your office). It is a good rule of thumb to allow two business days to make the changes to the network and GroupWise according to your request. When adding or moving users, submit your requests as many days ahead of time as possible; AVOID waiting until the day the user arrives in your office.
5. User's Last Name -- Type in the user's last name. This is required for GroupWise and network identification.
6. User's First Name -- Type in the user's first name. This is required for GroupWise.
7. User's Middle Initial -- If the user has a middle name, the initial is required, primarily for network identification.
Note: Items 8 and 9 pertain only to "Add" and "Move" requests.
8. User located in HSB -- This question applies only to users who will work in the Human Services Building in Salem on a regular basis and who will require more than a generic agency login ID at the workstation. The default is "No." Select "Yes" if the user will reside in the HSB.
9. ORGOVT post office -- This question applies to users who come to DHS from another state agency that uses GroupWise. ODOT and the State Fire Marshal are two prime examples. The default is "No." Select "Yes" if the user has an entry in the GroupWise address book with an ORGOVT domain listing.
10. Type of action and Action Section link -- Select the appropriate action by moving your mouse pointer over the appropriate "radio button" and clicking once. You can select only one action. Help is available online to advise you as to the best course of action; simply select the "Help" button in this section or the button at the top of the form. To finish completing the form, use the underlined link to jump to the section of the form that corresponds to the action you choose.
