Employee

When a potential conflict of interest is identified, complete a DHS 0429, Notice of Conflict of Interest with a Child Support Program Case, and submit copy to your immediate supervisor.

2. Supervisor

Review the DHS 0429 submitted by the employee and determine whether a conflict of interest exists.

If a conflict does not exist, send a copy of the DHS 0429 to the Office of Human Resources.

If a conflict does exist:

  1. Complete the manager’s section of the DHS 0429 and send a copy to the Office of Human Resources; and
    1. When the conflict is reported by an SPD employee, send a copy to SPD (Tishri Whiteside).


    2. When the conflict is reported by a CAF employee, send a copy to CAF (Security-Request, CAFRACF or David Stauffer).
3. Office of Human Resources File the DHS 0429 in a centralized Conflict of Interest file.
