When a potential conflict of interest is identified, complete a DHS 0429, Notice of Conflict of Interest with a Child Support Program Case, and submit copy to your immediate supervisor. |
| 2. | Supervisor | Review the DHS 0429 submitted by the employee and determine whether a conflict of interest exists. If a conflict does not exist, send a copy of the DHS 0429 to the Office of Human Resources. If a conflict does exist:

| 3. | Office of Human Resources | File the DHS 0429 in a centralized Conflict of Interest file. |
