General response and reporting:

1.

All employees and volunteers

Report to supervisor, or as an alternative, the Office of Human Resources, conduct or actions prohibited by policy.

Employees of DHS institutions are to follow internal procedures regarding reporting of incidents involving patients or clients.

2.

Supervisor

Take immediate action necessary to protect employees, volunteers, and building occupants including, if determined, taking action to have the offending individual removed from the work site as quickly as safety permits.

Conduct a prompt investigation of reports or rumors of a policy violation and initiate a timely and appropriate response.

Notify supervisor, other program management, and Centralized Threat Assessment Team (CTAT) per instructions in the Emergency Action Handbook.

If perpetrator is an employee: Immediately notify the assigned Human Resource Generalist.

When perpetrator is other than an employee: Appropriate actions may include suspension or termination of relationship with department, being trespassed from DHS worksite(s), suspension or termination of volunteer status, reassignment and/or referral to appropriate law enforcement authorities, which may result in criminal charges.

  • Trespass letters must be approved by DHS Safety & Health.
  • When an individual who has violated this policy leaves the premises and then returns prior to formal actions being taken, police shall immediately be contacted.
  • When a visitor brings a weapon into a DHS worksite, law enforcement shall be notified even if the person indicates they have a concealed handgun permit.

Voluntary:

Inform the supervisor, or as an alternative, Office of Human Resources if involved in a non-work related situation that may pose a risk to the workplace.
2. Supervisor Obtain the employee’s consent before notifying staff i.e., receptionists, building manager, other workers; etc. determined necessary to carry out a safety plan.

3.

All employees and volunteers

Mandatory:

If knowledgeable that a perpetrator intends, or has made threats to carry out an act of violence at a DHS worksite, there is an obligation to report per "General response and reporting" procedure, step #1 above.

If a DHS worksite is listed in a protective order, immediately submit to supervisor a copy of the order. As an alternative, employees may report to the Office of Human Resources.

3. Supervisor Report per instructions in the Emergency Action Handbook and to assigned Human Resource Generalist.

  • Threat Matrix - Provides an overview of steps to take in response to threats and acts of violence and weapons violations at DHS worksites. It is recommended that the matrix be posted on bulletin boards and near phones.
