1.

Manager

Determine and review need for a Group/Job ID.

Complete all areas of the Group/Job ID Exceptions Request form including the business justification, names of the Group/Job logon users, and their role(s).

Submit the Exceptions Request form to the Information Security Officer for approval.

2. Information Security Officer

Review request.

Approve or deny. Report decision to requesting manager.

If the exception is for a Group/Job ID for network logon, forward the approved request to OIS/ Help Desk (Network Administrator) for implementation.

If the exception is for a Group/Job ID other than the network, (RACF, TRACS, Oregon Access, etc.) forward the approved exception to the appropriate Chief Data Steward for implementation.

Assign a unique Group/Job identification.

Assign a password to be used only by the approved Group/Job users.

Notify requesting Manager and Information Security Officer of new Group/Job ID.
