Application Process

The General Assistance program is a benefit available to OSIPM Presumptive Medicaid individuals who are homeless or who are losing their housing within 90 days. Potential GA individuals can be referred to the GA Program using the Disability Benefits Liaison-General Assistance Referral form (DE 4640). A separate GA application is not required.
